In a series of 22 patients, Pluymaat et al (2013) showed that using an endoscopic suturing device to reduce the diameter of the GJA improved postoperative outcomes. This was an FDA-approved, multicenter trial that included 55 patients who underwent revisional endoscopic dilatation for successful weight loss with weight regain. Technical success was achieved in all patients (100 %). Mean anastomosis diameter was reduced to 10 mm (range of 5 to 16 mm). The mean weight loss at 3, 6, and 12 months was 6.5 kg, 10 kg, and 8.8 kg, respectively. After 6 months, the mean weight loss was significantly higher in the closed endoscopic group (mean of 12.6 kg) than in the open surgical control group (mean of 6.4 kg) (p = 0.04). There was no difference in outcomes between open and laparoscopic procedures. There were no major complications. The authors concluded that closed endoscopic surgery provided high levels of weight loss comparable to open surgical techniques in the short term.

In a study of 26 patients, Balaban et al (2014) showed that a combination of endoscopic suturing device and transmural myotomy by the same procedure did not provide better outcomes than suturing device alone. This was a prospective, single-center, cohort study of 26 consecutive patients who underwent ENDO-RE for weight regain after BPD-DS. Retrospective chart reviews were performed to capture demographic data, primary surgery, and early and late clinical outcomes. The primary outcome was complete weight loss at 1 year. Patients were evaluated at 1 year after ENDO-RE based on serum leptin, anthropometric, and metabolic variables. Weight loss at 1 year was 27.3 16.9 kg. Completion of the suturing procedure required approximately 20 min per patient. At 1 year, 23.3 % of patients had persistent GJ enlargement and 46.2 % of patients had required at least 1 revisional procedure. The authors concluded that ENDO-RE represents a safe and well-tolerated procedure to achieve a weight loss of approximately 27 kg over 12 months.
